First treatments require a consultation form which must be completed and returned prior to your appointment no less than 48 hours before your appointment (please see T's & C's). Consent form can be signed in house.
The consultation form will be emailed to you five days prior to your appointment. If you book within five days of your appointment your consultation form will follow shortly after your confirmation email.Please note your treatment may not proceed if forms are incomplete/ completed incorrectly.
